To the Greenwich District Board of Works. Gentlemen, In this, my Sixth Annual Report, I have again the honor of laying before you, in the annexed Tables, such general information as will enable you to examine the sanitary condition of your District during the past year of 1861, and also to make comparisons between that and the five preceding years. In the first four Tables is shown the number of Births and Deaths, the ages at which these Deaths took place, and the principal Diseases which caused them. The first quarter of the year, as will be observed on referring to Table No. 1, commenced with a somewhat excessive mortality, which excess appears to have shown itself more particularly amongst aged persons, 159 having died above the age of 60 years; 49 above the age of 80; and 6 above the age of 90 years. At the same time it was observed that the heavy mortality amongst these aged persons resulted, in a great measure, from Chest Diseases, which diseases had reference to the severe weather and low temperature at that season. As the season advanced and the temperature increased, the mortality, as seen in Table No. 2, was sensibly diminished, chiefly amongst persons at the period of life above referred to, and attributable greatly to the mitigation of the excess of Chest Diseases amongst such persons. 7 Then comes the period of the year when Epidemic Diseases usually make their appearance, and more especially Diarrhœa. On referring to Table No. 3, it will be seen that the Deaths from Diarrhœa and Dysentery amounted to the large number of 84, being an increase of 78 deaths from that disease over the preceding quarter. Chest Affections were materially diminished during that quarter. My observations refer more particularly to Epidemic and Chest Diseases, because it is a positive fact, that to those diseases must be ascribed annually nearly two-thirds of the whole mortality of this District. During the third quarter the Deaths from Epidemic Diseases began to show that a serious atmospheric influence was at work. Scarlet Fever numbered 29 deaths, Hooping Cough 23, Typhus Fever 24, and Diphtheria 17, which is the largest number of deaths registered as having arisen from that disease in this District during any quarter of the year since the disease first made its appearance in this country. During the last quarter of the year, as will be seen on referring to Table No. 4, the Deaths reached the large number of 621, being 46 above the number of Deaths registered during either of the previous quarters of the year. This increased Death-rate was still due to Epidemic influence: Scarlet Fever numbered 51 deaths, Hooping Cough 16 Diarrhoœ 21 (an unusual number from that disease at that period of year and at a reduced temperature), Typhus Fever 22, Diphtheria 15, whilst Phthisis, Heart and other Chest Affections, were again greatly on the increase, 203 having died from those last-mentioned diseases during that period. The Deaths from Croup, comparatively numerous, showing 13 from that disease; whilst Convulsions and other Brain Diseases carried off 80 persons during the last quarter of the year. It is also, perhaps, worthy of remark, that as Epidemic Disease began to spread and exert its influence on the health of the District, as shown in the annexed Tables, the mortality amongst persons of advanced years was perceptibly diminished, and as perceptibly increased at the same period amongst the infantile population, 493 children having died 8 under the age of 5 years during the last two quarters of the year, whilst only 275 died above the age of 60 years during the same period, those infantile deaths having been chiefly caused by Epidemic and Chest Diseases. Next in succession that I have to bring under your notice, will be Table No. 5, wherein is condensed the four Tables previously referred to, showing the total number of Births and Deaths, the ages at which those deaths took place, and the different diseases causing those deaths. It is not necessary to make any separate observations with reference to this Table, as the remarks to be made on the following Table, No. 6, in which a comparative statement is also given of the sanitary condition of the District during the past five years, will be applicable to both. The first fact to be observed in these two Tables will be the large number of Births during the year, amounting to 3121 throughout the whole District, and being 394 above the average of the four preceding years. The Births took place in the following numbers in the several Parishes of this District, viz. Greenwich 1332; St. Paul, Deptford, 1458; and 331 in St. Nicholas, Deptford. The next observation to be made on referring to that Table will be the large number of Deaths during the year, as compared with previous years, numbering 2239. These figures show an increase of 217 above the average of the four preceding years. Then comes the heavy infantile mortality which is so particularly remarkable. This fact, in relation to the sanitary state of the District, has been prominently brought to your notice in each of my former Annual Reports, and I have this year specially to draw your attention to a still heavier infantile mortality than has ever occurred in the District since the passing of the Metropolis Local Management Act; 846 children have died during the year under the age of 5 years, and 82 between the ages of 5 and 10 years, making a total of 928 deaths among children under 10 years of age, and being 145 deaths above the average of the four preceding years among children under that age. This, Gentlemen, is a 9 serious item in the Death-rate of the District, when it is seen that out of a total of 2239 Deaths during the year, 928 have died under the age of 10 years. The subject therefore requires serious consideration, and more particularly with reference to your future sanitary operations. On a further comparison of the past with preceding years, as regards the period of life at which the deaths have occurred, very little variation will be observed; and the same remark will apply to the causes of death, with the exception of deaths arising from Epidemic Diseases, which will be seen to have varied considerably during the past five years. The year 1861 has numbered a much larger proportion of Deaths from Epidemic influence than any preceding year with the exception of the year 1859, which also bore a heavy Epidemic Death-rate. Table No. 7 is so arranged as to show at a glance the number of Deaths occurring in each Division of the District during the past year, and at the same time a comparative statement of the Deaths as they severally occurred in each Division during the past six years. In Table No. 8 is shown the Death-rate per 1000 of the population. In the first column the Death-rate is calculated for the several Divisions of the District, including Greenwich Hospital and the Dreadnought Hospital and the Union House; and in the second column is a similar calculation exclusive of those Hospitals and the Union House. Table No. 9 shows the Births that were registered in the several Parishes of the District during the year, and also a comparative statement of the Births registered during the past six years. From this Table it will be seen that there were 303 more Births registered during the last than in the previous year, and 392 more than the average of the preceding five years. I have so arranged Table No. 10 as to point out the total number of Births and Deaths in each Parish of the District, and the excess of Births over the Deaths. From this Table will be observed the increase of the population of the District as regards the excess of Births over the Deaths. These figures do not show the real numbers or increase of the 10 population during the past year, inasmuch as a large number of the inhabitants is annually flowing into different parts of these towns from London and other localities, which renders it impossible to arrive at a correct conclusion. For further information and more particular details, I beg to refer you to the Tables above alluded to. Having presented you the annexed Tables, and having directed your attention to the increased mortality of this District during the past year, and the apparent causes of that mortality, it will no doubt be expected that I should offer some strong remarks thereon. In many of my Reports during the last and former years, I have so pointedly referred to the apparent causes of disease, and so repeatedly directed your attention to the remedies most conducive towards the improvement of the condition of the District, that if I were now again to enter into those details I should be only laying before you an epitome of what has been already elaborately described. I shall therefore confine myself to a few observations respecting the Drainage of this District. I have in many of my former Reports, in alluding to the causation of disease and particularly to those causes whereby diseases, at first simple in themselves, have, by direct atmos pheric or effluvial influence, been rendered malignant and even fatal (causes which could be really and sensibly prevented), directed your attention more especially to such specific influences as arise from want of drainage throughout the greater part of the District. On those several occasions it has been decided by your Board that it was not expedient to commence any system of drainage until the Metropolitan Board had perfected their scheme for intercepting the sewage of the Metropolis. That decision, as shown at a late meeting of your Board, now no longer prevails, and it has afforded me much satisfaction to find that some steps have at length been taken towards the removal of the chief and principal causes of the heavy mortality in the District; and, in alluding to the Resolution of your Board for appointing a Committee to enter into arrangements for obtaining and preparing plans and laying down a 11 system of drainage for the whole District, I wish particularly to refer you to an observation which fell from the Proposer of that Resolution: viz: "That after having obtained and fixed on Plans for a proper system of drainage, the Board would not be compelled to commence the drainage of the whole District simultaneously, but that a certain part or parts might be commenced at a time, and so gradually advance to the completion of the sewage of the District." That observation very exactly applies to some remarks which I had occasion to make to you in an early Report, and to which I am now more than ever desirous to direct your attention. There are some parts of the District in which a system of drainage can be immediately carried out; and I am particularly anxious, taking into consideration the severe and fatal Epidemic which has lately visited a certain part of this District (I am alluding more particularly to that part of the Parish of St. Paul, Deptford, known as the New Towr; and lying South of the Broadway and West of the Mill-lane sewer), that the drainage of that portion of the District should be commenced as soon as proper plans for which drainage can be determined on, and the necessary funds obtained. I would not again have urged this matter upon your notice but for the severe epidemic mortality which has of late occurred in those parts of the District above mentioned, and the fact of your having an outfall sewer in Mill Lane, into which those streets may be drained; an outfall sewer which, I believe I am correct in stating, must be made available for the drainage of that part of the town at whatever time it may be carried out. With these reasons and facts before me, I cannot too strongly press upon you the necessity of at once commencing the sewage of the greatest and worst portion of the New Town, Deptford. Such an operation will at once hasten the good work you have begun; such a determination will not interfere with the future plans for the drainage of the District; and such a decision will undoubtedly be the means of preventing a recurrence of such fatal outbreaks of disease as 12 have been lately too sensibly witnessed in those parts of the District to which I have above particularly adverted. During the past year about the usual amount of sanitary works has been carried out by your Officers and under the directions of your Board. About 2200 houses have been inspected throughout the District; the various Manufactories have been continually inspected and reported on; 747 privies and cesspools have been emptied and cleansed; 140 nuisances of other kinds have been removed, such as the keeping of pigs, accumulations of filth, dung, and dust heaps, &c. A large number of houses have been cleansed, lime-washed, and repaired, such as the houses in Collier Street, Deptford, Holdsworth's Rents, Deptford, Queen Street and East Street, Greenwich, and many other houses in various parts of the District, but the exact number of which I have not been able to ascertain. Many streets have been cleansed and paved, such as Blackhorse Fields, Deptford, &c., for a statement of which, and also a statement of the sewage, drainage, and water supply to the District, I must refer you to the Tables accompanying the Surveyor's Report. The slaughter-houses, rag-shops, and places of trade likely to become injurious to the public health, have been regularly inspected, and the usual amount of surface cleaning of the streets has been daily attended to by the Officers of the Board.
